The seven pilot states for Better Choices, Better Health are: California, Hawaii, Iowa, Maine, Massachusetts, New Jersey, and Oregon.
Better Choices, Better Health is the new online version of Stanford University's Chronic Disease Self Management Program (CDSMP). This six-week, interactive workshop is designed for people with a range of chronic health conditions, including diabetes. It is free for people in seven pilot states, thanks to a grant from The Atlantic Philanthropies.
A free Better Choices, Better Health Workshop can help you get the support you need and find practical ways to deal with pain, fatigue, and stress. You'll discover better nutrition and exercise choices, understand new treatment options, and learn better ways to talk with your doctor and family about your health.
If you have conditions such as diabetes, arthritis, high blood pressure, heart disease, chronic pain, or anxiety, a Better Choices, Better Health Workshop can help you get back in charge of your life.
The no cost Better Choices, Better Health Workshop is held entirely online and enables you to participate from any computer with an Internet connection.
You'll join up to 25 others in an interactive workshop and participate in easy-to-follow online sessions, which are posted each week for six weeks. You'll log on at your convenience 2-3 times per week for a total of about 2 hours per week.
You'll learn from trained volunteer leaders, many with health conditions themselves.
You'll set your own goals and make a step-by-step plan to improve your health-and your life.
Workshops can be accessed from any computer with an internet connection-including dial-up-making them accessible for those without access to transportation or broadband.

The National Council on Aging has licensed the program from Stanford University, but workshops are open to individuals of all ages.
