Greetings to you all.

I am Zahir shamsery ,from Bangladesh.

Age 62 years.

I was suspecting cancer in my tongue and went to cancer hospital at Kolkata, India.

After check up , Doctor assure me that it is not cancer.

But I have just been diagnosed with diabetes.

Those who have little idea about my country, know well that treatment in our country is out of the reach for lower middle class and middle class people.

For the low income group or working class people, nothing to mention , they have just born to die, I am sorry and ashamed but that is the reality.

Anyway, I wish to know, how do you differentiate the diabetes?

What would be the proper measure for a diabetic patient?

How I can take self care?

On receipt the reply shall remain grateful and obliged.

Thanks,

Zahir Shamsery.

Dhaka,Bangladesh.

dear Zahir: in the begining you should now that there are two main type of dibetes: 1- type 1 diabtes mellitus which is usualy occur in young. in which there pancreas will not produce enough insulin or produce no insulin at all. this type of patient they need insulin and they may go to something called ketoacidosis or die if they did not use insulin. 2- type 2 diabetes mellitus which is usualy occur in adult. this occur as the pancreas become tired and produce less insulin than body need or there is resistance in the body to the insulin. the main way of the treatment is diet control and exercise.then tablet and insulin. even though some of the patient will be started on insulin immediately if their blood sugar is very high. this type of diabetes usualy occur in overweight patient or obese. now what is the symptoms of diabetes: 1- weight loss without dieting 2- thirsty 3- excessive urination 4- waking up from the sleep to pass urine 5- impotance in male some people with type 2 diabetes will have no symptoms! now how you can take care of your self: 1- diabetic patient should follow up diabetic doctor or even General practionar regulary.they need to check patient sugar regularly and perscribe the suitable medication. 2- he should ask his doctor about a proper diet because it is the main core of diabetes management.some diabetic patient think that if they avoid sweet they will be better but this is not true because many type of food will be transform to sugar in our body after we eat it such as rice, bread and fruit. the idea is the doctor or the dietitian should explain to the patient about proper diet. it should be balanced diet contain suitable amount carbohydrate , protein and little fat. 3- patient should do some exercise---- at least to walk half an hour daily. or one hour three days per week. take care of your foot and if you have heart problem you need to ask your doctor before you initiate any type of exercise. 4- you need to see eye doctor once a year as diabetes can affect the eye even though patient will not feel it. 5- you should avoid smoking as it will affect the blood vessels and the heart. 6- keep you blood pressure under control ( in diabetic patient should not exceed 130/ 80) 7- always take care of your feets. check it regulary for injuries and craks if there is any injuries you need to see the doctor to avid any complication later. I think you have type2 diabtes mellitus which usualy depend on life style modification, balanced diet, exercise and medication ( tablet and insulin if need it)
